    Asthma is a chronic lung condition causing airway obstruction. Children with asthma may experience wheezing, coughing, chest tightness and trouble breathing. These symptoms are triggered as a result of the body’s reaction to environmental changes such as allergens or changes in temperature.

    Asthma is a chronic condition, controlling a child’s asthma symptoms at home helps to reduce the risk of further attacks.  Knowledge of the disease and its treatment is the key to good asthma control.  Asthma managed ineffectively may lead to frequent hospital visits.  Medications can prevent and reduce the number and severity of attacks.  It is important for children with asthma and their caregivers to know how to properly manage asthma symptoms and prevent asthma attacks from occurring.

    The Home Management Plan of Care helps children with asthma and their caregivers develop a plan to manage the child’s asthma symptoms and to know when to take action. It should address all of the following:

    1. Arrangements for follow-up care
    2. Environmental control and control of other triggers
    3. Method and timing of rescue actions
    4. Use of controller medications
    5. Use of reliever medications

    At SBUH, 58% of 48 eligible patients received a home management plan of care document.

    A high score is better than a low score.

    The SBUH 2009-20010 score is for the period October 2009 to September 2010.
    The New York State average score summarizes the average of health care organizations throughout New York State reporting to the Centers for Medicare and Medicaid Services (CMS). Currently, the New York State average scores have not been compiled.
    The U.S. average score summarizes the average of health care organizations throughout the United States reporting to the CMS for the period October 2009 to September 2010.
    Top 10% score summarizes results from the best 10% of health care organizations that are members of the University HealthSystem Consortium (UHC).
