About Us
Stony Brook Extended Care includes a primary care internal medicine practice and an obstetrics and gynecology practice. Our primary care practice provides preventive medicine and medical management of acute and chronic conditions to patients 18 and older. Our obstetrics and gynecology practice is devoted to the care of female patients, whether for regular annual exams, prenatal care or medical problems. Office visits, consults, as well as tests and procedures can be completed at our Centereach or Riverhead office.
Internal Medicine
Our physicians are board-certified internists serving the Long Island community for more than 15 years. Dr. Chawla and Dr. Kochar provide comprehensive medical care for adult patients with acute or chronic illnesses. With a focus on prevention, our physicians provide quality medical services in an ambulatory setting and in a timely manner. Our mission is to improve all patients' health, which means returning patients to the best possible wellness outcome through evidence-based treatment and a partnership to meet health goals together. We believe that optimum health includes meaningful patient and family health along with wellness education.
Obstetrics and Gynecology
Our obstetrics and gynecology specialist will provide patients with expert healthcare through all stages of a woman’s life. Dr. Jeremias and staff are dedicated to meeting your needs in our pleasant, convenient office setting. She is focused on routine health screening for women of all ages; low and moderate risk prenatal care and office-based management of a variety of gynecologic diseases and surgery.
Physicians & Staff
Dr. Chawla completed his medical school training at the University of Mumbai in India in 1983. He completed his internal medicine residency at Hahnemann University Hospital in Pennsylvania in 1989. He is board certified in internal medicine and has been in practice on Long Island since 1990. He has also taught students and residents at Stony Brook University Hospital. |

Dr. Kochar completed his medical school training at BJ Medical College in Ahmedabad, India. He completed his internal medicine residency at Upstate University Hospital in Syracuse, NY. He is board certified in internal medicine and has been in practice for 23 years. |

After completing her residency in obstetrics and gynecology at the University of Massachusetts Medical School, Dr. Jeremias joined Fallon Clinic, a local multi-specialty group practice. When her family moved to Long Island, Dr. Jeremias joined the obstetrics and gynecology division of Queens Long Island Medical Group. She has been providing care in Suffolk County since 2013.
